Initial Comments:
Based on the findings of an onsite unannounced followup survey conducted at the extension location (Vandergrift) on 5/24/19, found Westarm Therapy Services had corrected the deficiencies cited under 42 CFR, Part 485, Subpart H, Conditions of Participation for Clinics, Rehabilitation Agencies, and Public Health Agencies as Providers of Outpatient Physical Therapy and Speech-Language Pathology Services. The Immediate Jeopardy was abated on 5/24/19 at 9:50 a.m., based on full implementation of the facility's plan as evidenced by observation at the extension site location (Vandergrift), interviews with staff and review of facility documentation. The Immediate Jeopardy and associated deficiencies were cited as a result of a Medicare recertification survey completed 5/21/19.
